    Patient-Centered Medical Home (PCMH)

    Vibrant Health is excited to announce Patient-Centered Medical Home (PCMH) recognition for our third clinic, Children’s Campus. With this recent award, Vibrant Health now has PCMH recognition for all three clinic locations.

    PCMH is a set of standards set by NCQA (National Committee for Quality Assurance) that address access, knowing and managing patients, care coordination and quality. PCMH establishes a continuity of care so our high need and high risk patients have everything they need in one spot. This reduces medical costs by encouraging patients to establish a medical home with Vibrant Health instead of visiting the ED.

    To achieve recognition, Each Vibrant Health clinic passed all 40 core criteria and earn 25 elective credits across all concept areas:

    • Team-Based Care and Clinic Organization: Provides guidance on the structure of clinic leadership, care team responsibilities and how we partner with patients, families and caregivers.
    • Knowing and Managing Your Patients: Sets standards for data collection, medication reconciliation, evidence-based clinical decision support and other activities to ensure our providers know and understand our patients’ needs. It’s important for us to know our patient population and have an understanding of social determinants of health and what their barriers are to accessing health care.
    • Patient-Centered Access and Continuity: Guides clinics to provide patients with convenient access to clinical advice and helps ensure continuity of care.
    • Care Management and Support: Helps clinicians set up protocols to identify patients who need more closely-managed care.
    • Care Coordination and Care Transitions: Ensures that primary and specialty care clinicians are effectively sharing information and managing patient referrals to minimize cost, confusion and inappropriate care.
    • Performance Measurement and Quality Improvement: Helps practices develop ways to measure performance, set goals and develop activities that will continuously improve performance.

    It’s a 6-12 month process per clinic to be recognized as a Patient-Centered Medical Home. There are 89 sites in Kansas, 3 of them being Vibrant Health locations.

    Vibrant Health is proud to maintain PCMH recognition across all our clinics. The recognition puts patients first, allowing them to easily access quality health care when they need.
